Job Overview

Camber, a subsidiary of KVC Health Systems, offers comprehensive psychiatric inpatient treatment services for children and adolescents in a safe and secure environment.

We are seeking a skilled Case Manager to join our team and provide exceptional support to our patients and their families.

Key Responsibilities:

  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on mental health topics and treatment options.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Monitor patient progress and make adjustments to treatment plans as needed.

Requirements:

  • Bachelor's degree in a human service field, such as social work, education, sociology, psychology, counseling, or applied behavioral sciences.
  • Minimum of two years' experience working in case management, utilization review, or wellness coordination, with at least one year of experience working with economically disadvantaged, vulnerable, or at-risk youth and/or adults.
  • Valid driver's license and auto insurance.
