About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our team at eTeam Inc. as a Chronic Care Specialist. This is a unique opportunity to make a meaningful difference in the lives of our patients and their families dealing with end-stage renal failure or chronic kidney disease.

Key Responsibilities
  • Deliver specialized, complex care to patients with multiple co-morbidities, requiring unique treatment plans and a broad range of nursing skills and knowledge.
  • Build long-term relationships with patients and their families in an intimate outpatient setting.
  • Work collaboratively with a small interdisciplinary team of clinicians, including technicians, nurses, dietitians, social workers, physicians, and the center manager.
  • Participate in quality improvement initiatives to improve patient health and meet or exceed CMS standards.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • At least one year of dialysis experience within the last five years.
  • Excellent communication skills to listen and communicate with patients and teammates.
  • Basic computer skills and proficiency in MS Word and Outlook.
  • Intensive Care Unit (ICU), Critical Care Unit (CCU), Emergency Room (ER), or Medical Surgical (Med Surg) experience.
Work Environment

Our RNs work in a fast-paced environment, overseeing up to 12 patients at a given time, and up to 3 Patient Care Technicians who assist with patient observations, measuring stats, and machine set-up. Shifts are typically 12 hours, 3 days/week, Monday-Saturday (closed Sundays).
