Care Transition Specialist Lead

Brigham and Women's Hospital is seeking a highly skilled Care Transition Specialist Lead to join our team. As a key member of our Care Continuum Management team, you will play a critical role in ensuring seamless transitions for patients from our hospital to community settings.
Key Responsibilities:- Coordinate long and short-term placements to extended care facilities, including rehabs, sub-acute, and chronic facilities.
- Develop and maintain relationships with community agencies, clinics, schools, and courts to ensure optimal resource utilization.
- Act as a consultant to patients, families, and hospital staff on placement processes and access to community resources.
- Establish homecare plans in conjunction with Case Managers and document progress in the medical record.
- Monitor quality of care in ECFs, home/community agencies, and report findings to the Program Manager.
- Bachelor's Degree in a related field and 3-5 years of experience in hospital discharge planning, long-term care facility, community health, or utilization review.
- Extensive knowledge of regulations, community organization, state and federal systems, medical terminology, and levels of healthcare.
- Strong interpersonal skills to interact effectively with various levels of staff, patients, families, and community.
- Ability to manage a variable workload, prioritize tasks, and work independently.
Tuesday - Saturday, 8:00am to 4:30pm or 8:30am to 5:00pm, rotating hour, hybrid schedule.
