Care Transition Specialist Lead Job Summary

The Brigham and Women's Hospital (BWH) is seeking a highly skilled Care Transition Specialist Lead to join our team. As a key member of our Care Continuum Management team, you will be responsible for performing Care Transition Specialist duties, as well as analytical, administrative, and escalation duties for MGB Post-Acute Capacity.

Key Responsibilities:

  • Coordinate long and short-term placements to extended care facilities, including rehabs, sub-acute, and chronic facilities.
  • Develop and maintain relationships with community agencies, clinics, schools, and courts to ensure seamless patient transitions.
  • Monitor quality of care in ECFs, home/community agencies, and report findings to the Program Manager.
  • Analyze operational data to evaluate performance and identify areas for improvement.
  • Participate in relevant planning meetings to provide input into practice and program needs.

Requirements:

  • Bachelor's Degree in a related field and 3-yr experience in hospital discharge planning, long term care facility, community health, or utilization review.
  • Extensive knowledge of regulations, community organization, state and federal systems, medical terminology, and levels of healthcare.
  • Strong interpersonal skills to interact effectively with various levels of staff, patients, families, and community.

Working Hours: Tuesday - Saturday, 8:00am to 4:30pm or 8:30am to 5:00pm, rotating hour, hybrid schedule.
