Resident Service Coordinator

We are seeking a highly skilled Resident Service Coordinator to join our team at NHE, Inc. As a key member of our staff, you will play a vital role in providing exceptional service to our residents and ensuring their overall well-being.
Key Responsibilities- Conduct comprehensive assessments of residents to identify their needs and develop personalized service plans.
- Connect residents with local resources and services, including healthcare, social services, and community programs.
- Provide education and support to residents on topics such as healthy aging, nutrition, and disease management.
- Collaborate with community partners to develop and implement programs that promote resident engagement and well-being.
- Monitor and evaluate the effectiveness of services provided to residents and make recommendations for improvement.
- Bachelor's degree in social work, gerontology, or a related field.
- Minimum of 2 years of experience in social services or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of local resources and services for seniors and individuals with disabilities.
