The Terraces of Phoenix, a HumanGood community, is seeking a highly skilled Clinical Liaison to join its Skilled Nursing team. As a key member of our team, you will play a vital role in generating business from referral sources to meet our revenue and census goals.

Key Responsibilities:

  • Develop relationships with community hospitals, physician offices, and community health clinics to drive business growth
  • Coordinate external professional relationships to achieve the highest possible occupancy within budgetary and regulatory guidelines
  • Collaborate with the Skilled Nursing team to ensure seamless care transitions and exceptional resident outcomes

Requirements:

  • Two or more years of experience in a related field, preferably in long-term care settings with geriatric residents, admissions, and marketing
  • Current or prior Admissions, Clinical, or Post-Acute Liaison experience within a skilled nursing facility preferred
  • Clinical license such as RN, LPN, RT, IT, or SLP preferred
  • Bachelor's Degree, Medical or Business education, or training preferred
  • Valid driver's license

  • Competitive salary range: $65,000 to $85,000 (DOE) plus commission
