Clinical Liaison Specialist

Touchstone Health Services is seeking a skilled Clinical Liaison to oversee the comprehensive assessment processes for individuals with Serious Emotional Disturbance (SED) and Severe Mental Illness (SMI). This role requires the ability to work collaboratively with internal and external entities to ensure the completeness and accuracy of assessment packets.
The successful candidate will possess a Master's degree in a related field, clinical licensure, and experience in mental health assessment and service planning. Strong organizational, communication, and interpersonal skills are essential for this role.
Key Responsibilities:
- Compile and maintain accurate and up-to-date SED and SMI packets for all applicable members.
- Collaborate with internal and external entities to collect necessary information, ensuring the completeness and accuracy of the packets.
- Provide constructive feedback to staff, ensuring that assessments are comprehensive and align with industry standards and organizational protocols.
- Work closely with clinical and case management teams to facilitate seamless communication regarding individual cases.
- Stay updated on relevant regulations, policies, and best practices related to SED and SMI assessments, service planning, and assessment.
Requirements:
- Master's degree in a related field (e.g., Psychology, Social Work, Counseling)
- Clinical licensure (e.g., LCSW, LPC, LMFT, LISAC)
- Valid Arizona Driver's License
- Must possess an AZ Fingerprint Clearance Card or be eligible for one
- Proven experience in mental health assessment and service planning
- Familiarity with relevant regulations and standards
- Strong organizational, communication, and interpersonal skills
- Proficient with MS Office products, email, internet, basic database navigation
- Knowledge of electronic health records (EHR) systems
- Ability to work independently and collaboratively within a multidisciplinary team
