Job Summary:

The Team Care Coordinator will assist with the day-to-day operations of the coordination of patient and staff visits, scheduling, IDG meeting scheduling, communication with field staff, and backup to medical records. The ideal candidate will have excellent administrative and clerical skills, with a strong attention to detail and ability to multitask.

Responsibilities:
  • Answer and direct incoming phone calls to appropriate team members
  • Backup for office staff for lunches and out of office
  • Backup for medical records
  • Maintain the medical supply closet
  • Work with Clinical Directors for staff changes, updates, and discharges
  • Attend department meetings
  • Adhere to dress code and work schedule
  • Assemble Admission Packets
  • Handle primary care office duties, including managing multiple schedules and running eligibility for patients
Requirements:
  • Be 18 years of age or older
  • Two years of experience in healthcare or medical industry preferred
  • Knowledge of office management systems and procedures
  • Demonstrated ability to organize efficiently and establish priorities
  • Excellent written and oral communication skills
  • Ability to handle stressful situations with tact and diplomacy
  • Valid NM driver's license and reliable transportation
Physical and Environmental Demands:

Light to moderate physical demands, including occasional lifting up to 50 lbs and driving in various weather conditions.

Work Hours:

Varying hours, typically Monday through Friday 8 am to 5 pm, with other hours required as needed.
