Job Summary:

The Community & Supportive Services Coordinator will be responsible for developing and implementing supportive service programs that address social and recreational needs of our residents. This includes assessing residents' needs, making referrals, and acting as a liaison with service provider agencies, local governments, and businesses.

Key Responsibilities:
  • Coordinate the implementation of supportive service plans and deliver needed services at assigned properties.
  • Identify, develop, and maintain networks of community-based supportive and social service agencies.
  • Develop a resource directory of providers for residents and management staff.
  • Provide general case management and referral services to residents, linking them to service providers and agencies in the community.
  • Prepare reports on supportive service activities and serve as a liaison with non-profit partners, the State's Housing Finance Agency, and other governmental/reporting agencies.
  • Promote on-site educational events, including health care, job search seminars, and life skills training.
  • Work closely with management staff to coordinate training of residents on resident and management responsibilities.
  • Set up volunteer support programs with service provider organizations in the community.
  • Improve knowledge and skills through participation in training sessions, annual service coordinator meetings, staff meetings, and other professional development opportunities.
Requirements:
  • Bachelor's Degree
  • 5 years of human service experience
  • Ability to travel between three different properties in York, East Hempfield, and Lancaster, PA
