Position Overview: The Lead Registered Nurse is entrusted with the provision of direct patient care.

Qualifications:
  • Completion of an accredited Nursing Program.
  • Active RN licensure in Ohio or Indiana, with the ability to obtain licensure in the other state.
  • A minimum of two years' experience in Medical/Surgical, Oncology, or community health nursing is required.
  • Experience in hospice care is preferred.

Key Responsibilities:
Deliver proficient nursing care to designated patients through effective symptom management, emotional support, educational guidance, and prompt response to the needs of patients and their families, including:
  • Regular Homecare Visits.
  • End of Life/Continuous Care.
  • Patient Referrals and Admissions.
  • Support in Care Centers.
  • Reviewing Patient Charts.
  • Additional duties as assigned.
  • Ensure patient and family services align with the established Plan of Care and physician orders. Collaborate with the patient's physician and/or Medical Director as necessary and initiate appropriate telephone orders for modifications in the Plan of Care.
  • Coordinate Interdisciplinary Group (IDG) planning for assigned patients.
  • Engage in IDG evaluations of the patient, leading to the creation, execution, and adjustment of the IDG Plan of Care.
  • Oversee and assess the care delivered by volunteers, hospice aides, and primary caregivers.
  • Facilitate seamless transitions of care by coordinating the admission of home-care patients to inpatient facilities as required.
  • Clarify hospice services and benefits concerning Medicare, Medicaid, and Private Insurance, and understand the application of Palliative Care Interventions and Advance Beneficiary Notices (ABNs).
  • Obtain and document orders from Medical Directors and Physicians when appropriate.
  • Request necessary Durable Medical Equipment (DME) for patients.
  • Conduct thorough initial nursing assessments.
  • Be available for evening and weekend shifts as necessary to address the needs of patients and families and support agency backup schedules.
  • Complete all required documentation in accordance with organizational policies, accreditation standards, and regulatory requirements.
  • Maintain clinical proficiency for assigned duties and procedures.
  • Additional responsibilities as assigned.
