Key Responsibilities
1. Collaborate effectively with interdisciplinary teams and external agencies to ensure comprehensive patient care.
2. Maintain thorough documentation of patient interactions, care plans, and interventions throughout the care process. - 25%
3. Foster open communication with medical personnel regarding patient care and agency operations.
4. Uphold the highest standards of professionalism and ethical conduct at all times, adhering to agency policies and procedures. - 25%
5. Conduct timely assessments and interventions, adjusting care strategies based on patient progress and condition changes. - 10%
6. Initiate discharge planning to address ongoing patient needs well in advance of discharge dates, ensuring continuity of care.
7. Safeguard patient confidentiality and manage an acceptable caseload in accordance with agency guidelines. - 5%
8. Complete all required documentation in compliance with agency standards.
9. Participate in quality improvement initiatives, including chart audits and related projects. - 5%
10. Be open to accepting additional responsibilities as needed to support agency objectives. - 5%
Minimum Qualifications
Education: Associate's degree in Nursing
Experience: Minimum of 1 year as a Registered Nurse
Licensure: Valid Registered Nurse license in South Carolina.
Additional Requirements:
- A valid driver's license with a satisfactory three-year motor vehicle record and proof of auto insurance.
- Driving is a requirement for this role, utilizing your personal vehicle.
- Reliable transportation is essential, and adherence to the Prisma Health Driver Safety Policy is expected.
- Successful completion of driver training is mandatory.
