- Develop and implement individualized care plans to ensure quality and continuity of care.
- Provide skilled nursing care to patients in their place of residence, adhering to infection control measures and OSHA guidelines.
- Supervise and provide clinical direction to Home Health Aides/Licensed Vocational Nurses to ensure quality and continuity of service.
- Ensure accurate and timely documentation of patient care and progress.
- Monitor assigned cases to ensure compliance with third-party payer requirements.
- Demonstrate commitment to professional growth, competency, and patient-centered care.
- Current State License as a Registered Nurse or Licensed Vocational Nurse.
- Current CPR certification.
- Ability to document electronically.
