Bilingual Stabilization Case Manager

Hildebrand is seeking a skilled Stabilization Case Manager to join our team. As a key member of our staff, you will play a critical role in disrupting the cycle of homelessness for families in greater Boston.
Key Responsibilities:- Support families exiting Hildebrand Emergency Shelter for 24 months.
- Develop and implement individualized stabilization plans, tracking progress and updating records as needed.
- Establish and maintain effective relationships with landlords, advocating for clients as necessary.
- Provide case management services, including assistance with securing medical services and benefits.
- Collaborate with colleagues to ensure comprehensive support for clients.
- Associate's degree or Bachelor's degree preferred.
- 3-4 years of experience in case management, housing search, or related work with homeless or low-income families.
- Detailed understanding of Massachusetts benefit programs, state and federally funded housing programs, and the social service network.
- Ability to work effectively with diverse populations and communicate effectively verbally and in writing.
- Bilingual in Haitian Creole required.
- 20 hours/week
