Job Title: Case Manager

SOS International LLC is seeking a skilled Case Manager to join our team in Cambridge, Massachusetts. As a Case Manager, you will play a vital role in supporting our customers in their journey towards stability and self-sufficiency.

Job Summary:

The Case Manager will be responsible for providing assessments, case management, and advocacy services to our customers. This includes conducting individual and group interviews, maintaining accurate records, and collaborating with community resources to ensure our customers receive the necessary support.

Key Responsibilities:
  • Maintain accurate and up-to-date records of customer interactions and progress.
  • Conduct assessments to identify customer needs and develop personalized plans to address those needs.
  • Collaborate with community resources to provide referrals and support to customers.
  • Provide advocacy services to customers, including assistance with accessing benefits and services.
  • Supervise and communicate with caregivers as needed.
  • Determine customer eligibility for social services and subsidies.
  • Conduct and document assessments in accordance with customer policies and procedures.
  • May arrange referrals, consultations, and therapeutic services as needed.
  • Reports significant incidents in accordance with policies and procedures.
Requirements:
  • Bachelor's degree in Social Work, Psychology, Sociology, or other relevant behavioral science, or four years of relevant case management experience in-lieu of a degree.
  • One (1) year of case management or other directly related experience.
  • Ability to act responsibly, professionally, and use good judgment under pressure.
  • Ability to multitask in fast-paced, deadline-driven environment.
  • Excellent interpersonal and staff management skills.
  • Strong verbal and written communication skills.
  • Proficient in Microsoft Office suite.
  • Must be authorized to work in the U.S.
  • Must pass background checks and Child/Adult Protective Services (C/AN) checks.
Preferred Qualifications:
  • Experience and/or training in shelters.
  • Experience working with immigrant populations and child welfare.
  • Bi-lingual fluency in Haitian Creole and English.
Work Environment:

SOS International LLC operates 24/7/365, and this role requires flexibility and adaptability. The successful candidate will be able to work evenings, weekends, and holidays as needed, and be willing to travel throughout the U.S. as required.
