Stabilization Case Manager

Hildebrand is seeking a highly skilled and compassionate Stabilization Case Manager to join our team. As a key member of our staff, you will play a critical role in helping families experiencing homelessness achieve stability and self-sufficiency.

Key Responsibilities:
  • Support families exiting Hildebrand Emergency Shelter for 24 months, providing guidance and resources to help them achieve stability.
  • Develop and implement individualized stabilization plans, tracking progress and making adjustments as needed.
  • Build and maintain strong relationships with landlords, advocating for families and resolving any issues that may arise.
  • Document all stabilization efforts and meetings, ensuring accurate and timely reporting.
  • Track and record client demographics, income, immigration status, and other key data points, providing valuable insights for program improvement.
  • Cultivate successful landlord-tenant relationships, providing mediation and support as needed.
  • Advocate for families in housing court appearances, ensuring their rights are protected.
  • Intervene if a tenancy is endangered, taking swift action to resolve the issue.
  • Assist families in securing medical services and benefits, connecting them with primary healthcare providers.
  • Develop and facilitate groups, classes, and skills training to help clients achieve their goals and objectives.
  • Measure and review case progress, facilitating case consultation meetings to ensure optimal outcomes.
  • Collaborate with Family Shelter Case Managers, Housing Specialists, System Navigators, supervisors, and other program staff to achieve shared goals.
  • Participate in weekly supervision, staff meetings, in-service training, and team meetings, staying up-to-date on best practices and program developments.
  • Be on-call, providing 24/7 support to families in need.
Requirements:
  • Associate Degree or Bachelor's degree preferred, with a combination of education and experience considered.
  • 3-4 years of experience in case management, housing search, or related work with homeless or low-income families.
  • Detailed understanding of Massachusetts benefit programs, state and federally funded housing programs, private housing developments, and the social service network.
  • Ability to work effectively with individuals from diverse backgrounds, communicating effectively verbally and in writing.
  • Computer literacy, familiarity with MS Word, and competence in utilizing software programs necessary for reporting requirements.
  • CPR/SFA certification within 30 days of hire.
  • Bilingual in Haitian Creole required.
  • Personal vehicle or ability to efficiently utilize public transportation for home visiting required.
