About the Role

We are seeking a highly skilled Patient Care Coordinator to join our team at Grane Home Health & Hospice. This role involves working closely with patients, families, and healthcare professionals to deliver high-quality care.

Job Description
  • As a Patient Care Coordinator, you will be responsible for planning, directing, and delivering safe patient care, ensuring the best possible outcomes for our patients.
  • You will collaborate with our interdisciplinary team to meet the individual needs of our patients and their families.
  • With your strong clinical skills and attention to detail, you will contribute to creating a supportive and caring environment for our patients.
Requirements
  • We are looking for a Registered Nurse with 1 year of experience in nursing, preferably in a hospice or similar setting.
  • You should have a strong understanding of the hospice philosophy of care and be committed to clinical and documentation excellence.
