Care Manager for Independent Living Services

A Bridge to Independence (ABI) is seeking a dedicated and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in supporting individuals with disabilities to live independently in their communities.
About the Role:
- We are offering an annual salary of $32,000, plus mileage reimbursement, making this an attractive opportunity for those looking to start or advance their career in the field of care management.

Key Responsibilities:
- You will be responsible for coordinating services for individuals with disabilities, including home and community-based services, to support their independent living goals.
- As a Care Manager, you will conduct regular phone calls and home visits to assess the needs of our clients and develop personalized service plans to meet those needs.
- You will also be responsible for completing intake forms, re-certifications, and other documentation as required by regulatory agencies.
Requirements:
- A bachelor's degree in a relevant field, such as social work, sociology, or psychology, with at least 12 college-level credits in one of these areas.
- Alternatively, you may have a combination of experience and training that adds up to 4 years of experience, along with education that includes at least 12 semester hours of college-level courses in a relevant field.
- Required training includes OLTL Service Coordinator training, with a minimum of 40 hours of training during the first year of employment and 20 hours annually.
- Experience in case management, service coordination, and/or HCBS program participant experience is highly desirable.
