We are seeking a skilled and compassionate Care Coordinator to join our interdisciplinary team serving adults with behavioral health challenges in Lewiston. As a Care Coordinator, you will play a vital role in providing individualized support and connecting clients with community resources.

Key Responsibilities:

  • Conduct intake and ongoing assessments to identify client needs and develop personalized plans of care.
  • Facilitate team meetings and collaborate with healthcare professionals to ensure comprehensive care.
  • Monitor client progress, document outcomes, and maintain accurate records.
  • Provide support and guidance to clients, their families, and caregivers.
  • Participate in ongoing training and professional development to enhance skills and knowledge.

Requirements:

  • MHRT/C certification or eligibility.
  • Bachelor's degree in a related field preferred.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Strong problem-solving and critical thinking skills.
