About the Role

We are seeking a skilled and compassionate Care Coordinator to join our interdisciplinary team serving adults with behavioral health challenges in Lewiston. As a Care Coordinator, you will play a vital role in providing individualized support and connecting clients with community resources to achieve their goals.

Key Responsibilities
  • Conduct intake and assessment of clients, developing a plan of care that addresses their unique needs and goals.
  • Facilitate individual service plan team meetings and collaborate with a multidisciplinary team to ensure comprehensive care.
  • Monitor and document client progress, making adjustments to the plan as needed.
  • Partner with clients' primary care providers and community professionals to ensure seamless care coordination.
  • Provide support and education to clients and their families, empowering them to make informed decisions about their care.
  • Identify and address physical and behavioral health-related risk factors, connecting clients with resources to mitigate these risks.
Requirements
  • MHRT/C or MHRT/C eligible.
  • Bachelor's degree in a related field preferred.
