At All Pointe Care, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Private Duty LPN - Client Care Coordinator. In this role, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.
Job Summary:The LPN will assist the Registered Nurse in carrying out the client's plan of care, preparing and implementing the client's care plan, and observing, recording, and reporting client symptoms, reactions, and changes. The LPN will also provide emotional and physical comfort and safety to clients, teach clients self-care techniques, and foster cooperative efforts among personnel.
Responsibilities:- Assist the Registered Nurse in carrying out the client's plan of care.
- Prepare and implement the client's care plan.
- Observe, record, and report client symptoms, reactions, and changes.
- Provide emotional and physical comfort and safety to clients.
- Teach clients self-care techniques.
- Foster cooperative efforts among personnel.
We offer a flexible schedule with three shifts: 7-3, 3-11, and 11-7. You will work 8-40 hours per week, depending on client needs.
