Job Summary

The Licensed Practical Nurse - Home Care Specialist provides skilled nursing services to patients in their homes, working under the direction of the RN Case Manager. This role requires a strong understanding of professional nursing theory and practice, as well as the ability to adapt to changing situations and prioritize tasks effectively.

Key Responsibilities
  • Assess the physical and psychosocial status of patients and develop individualized care plans
  • Provide intermittent skilled nursing services to patients and their families, including medication management and wound care
  • Document all services provided in the clinical record, ensuring compliance with state and federal regulations
  • Collaborate with the RN Case Manager to develop and implement care plans, and to address any concerns or issues that arise
  • Participate in ongoing education and training to maintain current knowledge and skills
Requirements
  • Graduate of NLN-approved school of licensed practical nursing
  • Current license to practice as a licensed practical nurse in the State of Connecticut
  • One year of recent medical/surgical nursing experience, with additional experience in community health nursing preferred
  • Ability to maintain quality control standards and react calmly and effectively in emergency situations
Working Conditions

The Home Care LPN - Skilled Nursing Professional works in patients' homes, community settings, clinics, and in an office environment. This role requires the ability to work under stressful conditions, with some exposure to communicable diseases and/or bodily fluids.
