Job Title: Licensed Practical Nurse

At All Pointe Care, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Private Duty Home Care provider.

Job Summary:

We are looking for an LPN who is dedicated to providing high-quality, personalized care to our clients in the comfort of their own homes. As a Private Duty LPN, you will work one-on-one with clients to develop and implement individualized care plans that meet their unique needs and goals.

Responsibilities:
  • Assist the Registered Nurse in carrying out the client's plan of care.
  • Assist with the preparation, implementation, and continuing evaluation of the client's care plan.
  • Observe, record, and report to the appropriate person client symptoms, reactions, and changes, including:
    • The general physical and mental condition of clients and signs and symptoms that may be indicative of untoward changes.
    • Stresses in human relationships between clients and staff and between clients and their families and visitors.
  • Prepare clinical and progress notes to be submitted on a weekly basis.
  • Recognize and understand the effects of social and economic problems upon clients and provide for the emotional and physical comfort and safety of clients.
  • Teach clients the appropriate self-care techniques and other aspects of daily living.
  • Foster cooperative efforts among personnel by understanding the functions of other persons involved in client care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and in-service programs.
Shifts and Hours:

We offer flexible scheduling to accommodate your needs, with available shifts including:

  • 1st shift: 7am-3pm
  • 2nd shift: 3pm-11pm
  • 3rd shift: 11pm-7am

We require a minimum of 8 hours per week, with the opportunity to work up to 40 hours per week.
