Key Responsibilities
- Develop and implement individualized case plans to address client needs and goals
- Conduct thorough assessments and identify resources to support clients
- Build and maintain relationships with community partners and service providers
- Provide crisis intervention and support services as needed
- Maintain accurate and confidential records of client interactions and progress
Requirements
- Bachelor's Degree in Social Work, Human Services, or a related field
- Experience in case management, working with families and individuals in need
- Strong understanding of case management and social work protocols and practices
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Excellent communication and interpersonal skills
