About the Role

Novant Health is seeking a highly skilled Care Connections Specialist to join our team. As a key member of our healthcare operations team, you will be responsible for ensuring seamless patient access and care coordination across our network of facilities.

Key Responsibilities
  • Manage patient scheduling, verification, and registration processes to ensure accurate and timely data entry.
  • Respond to patient inquiries and provide exceptional customer service, addressing concerns and resolving issues in a professional and courteous manner.
  • Collaborate with acute care partners to support critical work streams, including physician-to-physician consults, emergent transfers, and radiology code critical communications.
  • Serve as a resource to internal and external stakeholders, providing administrative and clerical support to ensure operational effectiveness.
  • Work collaboratively with cross-functional teams to identify and implement process improvements, ensuring a high level of patient satisfaction and care quality.
Requirements
  • High School Diploma or equivalent required.
  • Minimum one year of experience in customer service or customer relations, preferably in a healthcare setting.
  • Excellent interpersonal and communication skills, with the ability to provide remarkable customer service.
  • Ability to organize and prioritize work in a fast-paced environment, with a focus on attention to detail and time management.
  • Computer proficiency in Microsoft Office, with the ability to multitask and work in various software and internet platforms.
  • Working knowledge of electronic medical record systems and basic medical terminology.
