Responsibilities:

Conduct home visits to assess client needs and develop care plans
Advocate for clients' health and well-being through interventions and recommendations
Complete required documentation and ensure caregivers follow company protocols
Participate in creating daily client visit schedules and proactively fill schedule gaps
Provide education, training, and coaching to caregiving staff
Act as liaison between contract nurses, clients, direct care workers, and management

Requirements:

RN degree
Prior home care experience preferred
Prior supervisory experience helpful
