Job Summary

Catholic Charities Diocese seeks a skilled Housing Navigator Specialist to join our team. The successful candidate will assist participants in establishing and achieving housing goals, coordinating related services, and locating suitable housing options.

Key Responsibilities
  • Assist participants in establishing realistic, attainable housing goals
  • Engage in Diversion conversations and conduct VI-SPDAT assessments to connect participants with housing resources
  • Provide case management and coordinate care with community partners to assist participants in accessing services appropriate to their individual needs
  • Assist clients with obtaining essential documents and completing applications to obtain benefits, employment, or housing
  • Establish and maintain positive relationships with landlords and rental agencies offering affordable rental properties
  • Document all client contacts in ClubHOMS, Clarity HMIS, and Catholic Charities tracking systems
Requirements
  • Minimum of two years of experience providing case management for people who are unhoused
  • Minimum one year of experience serving people living with mental illness and/or substance use disorders
  • Associate degree in psychology, counseling, social work, or a related field or equivalent experience
  • Solid working knowledge of relevant best practices including Housing First, Low Barrier Services, Trauma-Informed Care, Motivational Interviewing, Diversion, Homeless Prevention, Progressive Engagement, Conflict Resolution, and Harm Reduction
  • Well-developed problem-solving skills and flexible, creative thinking skills with a positive, can-do attitude
  • Must have strong computer skills and the ability to become proficient with database software
  • Ability to read, interpret, and apply documents such as policies, procedures, and safety guidelines
