Lead Case Manager: Housing Navigation Services

We are seeking a highly skilled and compassionate Lead Case Manager to join our Housing Navigation Services team at PATH (People Assisting the Homeless). As a key member of our team, you will play a vital role in supporting clients in breaking the cycle of homelessness by providing individualized case management services.
Key Responsibilities:
- Provide oversight of program contracts to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services.
- Collaborate with the Quality Assurance Department to ensure programs are in compliance with contracts and partnership agreements.
- Function as a contact representative for program matters.
- Provide administrative oversight of the team, including coverage for the program manager.
- Support with the completion of critical incident reports as required per PATH policies and/or the contract.
- Conduct intake and individualized needs assessments for all clients and collaborate to develop Individualized Service Plans (ISPs) that address housing goals, maintaining/increasing income, and other personal goals identified by the client.
- Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
- Complete program-specific assessments, such as the VI-SPDAT.
- Evaluate strengths and challenges to addressing short-term and long-term goals, conduct 90-day reviews with clients after initial intake.
Requirements:
- Three (3) years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
- One (1) year of experience in healthcare, case management, homeless services, or working with vulnerable populations (minimum qualification).
- Employment eligibility verification.
- CPR/First Aid training.
- Background screening.
- Drug test.
- Reliable transportation.
- A valid driver's license.
- Proof of insurance and ownership for personal vehicles used during work duties.
- The ability to qualify for PATH's insurance coverage.
