Lead Case Manager: Housing Navigation Services

About the Job

We are seeking a highly skilled and compassionate Lead Case Manager to join our Housing Navigation Services team at PATH (People Assisting the Homeless). As a key member of our team, you will play a vital role in supporting clients in breaking the cycle of homelessness by providing individualized case management services.

Key Responsibilities:

  • Provide oversight of program contracts to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services.
  • Collaborate with the Quality Assurance Department to ensure programs are in compliance with contracts and partnership agreements.
  • Function as a contact representative for program matters.
  • Provide administrative oversight of the team, including coverage for the program manager.
  • Support with the completion of critical incident reports as required per PATH policies and/or the contract.
  • Conduct intake and individualized needs assessments for all clients and collaborate to develop Individualized Service Plans (ISPs) that address housing goals, maintaining/increasing income, and other personal goals identified by the client.
  • Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals.
  • Complete program-specific assessments, such as the VI-SPDAT.
  • Evaluate strengths and challenges to addressing short-term and long-term goals, conduct 90-day reviews with clients after initial intake.

Requirements:

  • Three (3) years of experience in healthcare, case management, homeless services, or working with vulnerable populations.
  • One (1) year of experience in healthcare, case management, homeless services, or working with vulnerable populations (minimum qualification).
  • Employment eligibility verification.
  • CPR/First Aid training.
  • Background screening.
  • Drug test.
  • Reliable transportation.
  • A valid driver's license.
  • Proof of insurance and ownership for personal vehicles used during work duties.
  • The ability to qualify for PATH's insurance coverage.
