Senior Community Integration Specialist

Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are seeking a Senior Community Integration Counselor in Trevose, PA.
This role is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills.
The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.
Responsibilities include:
- Assist individuals with daily living skills and the development of community involvement, communication skills, problem-solving skills, and social skills
- Provide ongoing support for community integration efforts
- Assist with developing a schedule for each participant to maximize community integration
- Provide training to individuals towards achieving program goals
- Complete training to facilitate staff development
- Administer medications and ensure proper completion of necessary documentation
- Plan, develop, facilitate, and participate in individual and group activities with participants, including leisure and volunteer opportunities
- Develop individual day program support plans
- Provide reports on individual progress in relation to overall goals
- Maintain inventory and ensures adequate supplies for program
- Oversee consumer funds as needed
- Monitor the health and medical needs of individuals and immediately report any concerns
- Drive individuals to medical appointments and activities in the community
- Ensure program documentation and billable records are completed accurately and timely
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
- Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation
- Additional tasks and responsibilities may be assigned
Minimum Requirements:
- High School Diploma or GED
- Bachelor's degree preferred
- Valid driver's license with a satisfactory driving record
- Minimum two years of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
- Excellent verbal and written communication skills
- Excellent time management skills
