The Clinical Nurse Specialist serves as a primary caregiver for a designated patient demographic, aiming to achieve optimal health outcomes.

This role emphasizes the enhancement of knowledge and skills, delivering effective direct care as part of a multidisciplinary team to a diverse range of complex patients.

This position supervises and guides the care provided by other healthcare professionals through delegation, validation, evaluation, and coordination. It also ensures effective collaboration of patient care and unit activities among nursing colleagues, physicians, and support services.

Assessment, Planning, Implementation, and Evaluation

Assessment

Gather comprehensive data relevant to the patient's health or situation.


Conduct age and disease-specific assessments/data collection independently in a systematic manner, focusing on physiological, psychological, and cognitive status.

Identify clinical and diagnostic changes in patient status.

Recognize variables in patient/family education needs based on age, disease, and culturally specific learning differences.

Plan

Develop a care plan that outlines strategies and interventions to achieve expected outcomes.

Formulate age and disease-specific patient/family-centered goals/outcomes based on assessment data.


Design and coordinate a care plan that is patient/family-focused, prioritized, and encompasses the continuum of care.

Implementation

Communicate, coordinate, and execute the care plan.


Maintain ongoing communication with patients/families, nursing colleagues, and other healthcare team members regarding the patient's status, care rendered, and treatment plans.

Implement age and disease-specific care in an organized and timely manner.

Provide or coordinate patient/family education as needed and ensure comprehension.

Adhere to established protocols in clinical situations, including communication with appropriate personnel and documentation of events.


Deliver nursing care and treatments that reflect the patients' needs, advocate appropriately, and demonstrate evidence-based nursing practice and compliance with standards.

Perform continuous Point-of-Care electronic documentation of each individual patient's assessments, care plan, interventions, and evaluation of care provided.


Administer medications utilizing the latest advancements in bar-scanning and documentation technologies designed to ensure the safest delivery of medications to our patients.

Facilitate multidisciplinary discharges of patients to ensure safe transitions and continuity of care.

Evaluation

Assess progress toward achieving outcomes.

Utilize critical thinking skills based on nursing knowledge to attain desired patient outcomes.

Initiate interventions related to changes in patient condition and unexpected responses to care.

Additional

Responsibilities:

Identify ethical issues in the practice setting and bring them to the attention of other team members.

Recognize and respond to opportunities to enhance patient satisfaction.


Must acquire and maintain competencies, certifications, and continuing education to meet any facility accreditation or individual licensing requirements.

Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.

Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand for a minimum of 6 hours a day.

Comply with HMH's Organizational and Managerial competencies and standards of behavior.

Qualifications:
Education, Knowledge, Skills, and

Abilities Required:
A minimum of an associate degree in Nursing or a diploma in Nursing.

BSN in progress and/or willingness to obtain within three (3) years of hire. All staff Registered Nurses (RN) are required to have a BSN within five years of hire.

Note:
Advanced master's degree in nursing programs satisfy the BSN requirement (i.e., Associates to MSN).


Must have six (6) or more months of relevant nursing experience OR must be an active participant in Hackensack Meridian Health's Nurse Residency Program.


The ability to learn quickly and adapt to changing patient needs, a strong sense of accountability for improving the lives of our patients and their caregivers, an exceptional focus on teamwork, dedication to ongoing education, and the ability and passion to deliver the highest quality of care based on a strong sense of patient focus are all required.

Education, Knowledge, Skills, and

Abilities Preferred:
National Certification in the area of specialty preferred when eligible.

Licenses and Certifications Required:
NJ State Professional Registered Nurse License.

BLS/CPR certification by the American Heart Association is required within 60 days of hire.
