Position Overview
The Clinical Nurse Specialist operates within a vibrant interdisciplinary team, delivering therapeutic services aligned with the Pinelands Recovery Center of Medford Clinical Model.
Key Responsibilities
1. Conducts comprehensive assessments for each prospective admission, evaluating medical suitability for facility admission and collaborating with medical and psychiatric providers for necessary admission orders.
2. Completes the initial nursing evaluation and oversees the full medical admission protocol for every patient.
3. Ensures the execution of all directives from medical and psychiatric professionals.
4. Verifies that all patients have provided informed consent for medications prior to the administration of the first dose.
5. Administers prescribed medications and monitors the effectiveness of PRN medications within designated timelines.
6. Demonstrates a thorough understanding of Axis I diagnoses and associated symptoms.
7. Develops and implements individualized Nursing Care Plans.
8. Adheres to narcotic diversion protocols as per established policies during each shift.
9. Engages in regular meetings with medical staff and the treatment team to discuss patient care, needs, and interventions.
10. Provides medical assessments as required.
11. Administers primary first aid and assesses the need for physician notification.
12. Conducts vital signs checks, basic evaluative procedures, and necessary tests.
13. Performs blood draws and manages NG tube maintenance, including administering bolus feeds when appropriate.
14. Offers health education to both patients and staff.
15. Responsible for ensuring that all required medications for patients are accurately ordered and readily available within the facility.
16. Manages and orders nursing supplies, maintaining a fully stocked and operational nursing station.
17. Communicates effectively and professionally with families and loved ones as appropriate.
18. Establishes rapport with patients in a professional manner.
19. Provides milieu management throughout the treatment process and collaborates with all team members.
20. Supervises and supports patients during community outings.
21. Accurately documents patient progress in accordance with policy requirements.
22. Assists with the discharge process.
23. Maintains a safe and organized environment.
24. Evaluates patient suicidality and assesses potential risks to self or others.
25. Communicates with referral sources and outpatient providers to ensure continuity of care during admission, midpoint, and discharge as necessary.
26. Makes timely requests for any absences and ensures adequate coverage for scheduled shifts.
27. Manages patients' medical needs, including scheduling appointments and completing referrals/prior authorizations for medical treatments and tests as indicated.
Additional Duties
1. Oversees the medication management system, which encompasses medication planning, storage, ordering, transcribing, administration, monitoring, and evaluation.
2. Monitors CIWA/COW scores for patients undergoing detoxification protocols.
3. Possesses knowledge of detoxification medications, withdrawal symptoms, and potential medical complications associated with detox.
This is a full-time overnight position.
