Social Care Specialist

Virginia Mason Franciscan Health is seeking a skilled Social Worker to provide social work interventions for priority Medical Center patient and family needs. As a key member of our team, you will contribute to the strength of our Team Medicine approach to collaborative medicine and benefit from the changes enacted through our Virginia Mason Production System.
Responsibilities
- Provide social work interventions for priority Medical Center patient and family needs.
- Work collaboratively with healthcare teams to develop and implement patient-centered care plans.
- Conduct psycho-social assessments and develop treatment plans to address patient and family needs.
- Provide education and support to patients and families on healthcare-related issues.
- Collaborate with community resources to provide patients and families with access to necessary services and support.
Requirements
- Master's degree in Social Work from an accredited college or university.
- Registration with the state of Washington as either a Licensed Social Work Associate (LSWAIC), Independent Clinical Social Worker (LICSW), Social Work Associate Advanced (LSWAA) or Social Work Advanced (LISW).
- Strong interpersonal and organizational skills.
- Working knowledge of individual and family dynamics and the ability to perform psycho-social assessments.
- Understanding of systems and how to work within systems.
- Understanding of the impact and implications of disease and the course of disease.
