Job Title: Travel Social Work Specialist

We are seeking a skilled Travel Social Work Specialist to join our team at Kindred Hospital - Seattle -First Hill. As a Travel Social Work Specialist, you will be responsible for providing high-quality social work services to our patients and their families.

Job Summary:

This is a travel social work position that requires a strong background in social work principles and practices. The ideal candidate will have experience working with diverse populations and be able to provide culturally sensitive care.

Responsibilities:

  • Provide social work services to patients and their families, including assessment, counseling, and advocacy.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Document patient progress and outcomes in accordance with hospital policies and procedures.

Requirements:

  • Master's degree in Social Work (MSW) or related field.
  • 2+ years of experience in social work, preferably in a hospital setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
