Job Summary

We are seeking a skilled Master Social Worker to join our team as a Patient Care Specialist. In this role, you will be responsible for providing social work assessments and interventions for hospitalized and emergency department patients.

Responsibilities
  1. Developmental Care Planning: Develop and implement care plans that address the unique needs of patients across all age groups, including those with physical, mental, and chronic health conditions.
  2. Advocacy and Education: Provide patient and family education on self-care management, health management, and bioethics, while advocating for patients' rights and needs.
  3. Patient Support Services: Offer crisis intervention, counseling, and referrals for patients and families, as well as support for addressing social determinants of health, such as housing and food insecurity.
  4. Interdisciplinary Collaboration: Participate in interdisciplinary care conferences to ensure seamless transitions of care and coordinate with healthcare teams to address complex patient needs.
  5. Community Resource Coordination: Develop and maintain relationships with community resources to support patients with complex needs, including those experiencing homelessness.
  6. Performance and Outcomes Management: Analyze and implement evidence-based practices to support organizational financial performance, length of stay, and readmission prevention efforts.
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work from a Council of Social Work Education accredited program.
  • At least 1 year of post-MSW experience in a clinical or medical setting, with a preference for 3+ years of healthcare experience.
  • Current licensure as a Licensed Advanced Social Worker (LASW:WA) or equivalent.
Benefits

We offer a competitive salary range of $85,000 - $110,000 per year, depending on experience, as well as benefits including health, dental, and vision insurance, FSA, matching retirement plans, paid vacation, adoption assistance, and more.
