Job Title: Social Services Specialist I

Sea Mar Community Health Centers is seeking a highly motivated and compassionate Social Services Specialist I to join our team. As a key member of our MSS Department, you will play a vital role in providing comprehensive integrated clinical psychosocial assessments, developing intervention plans, and providing brief treatment to clients in need.

Job Summary:

The Social Services Specialist I will conduct initial screenings, provide assessments, and develop care plans to address the complex needs of our clients. You will work closely with our interdisciplinary team to ensure that clients receive the best possible care and support. This is an exciting opportunity to make a meaningful difference in the lives of our clients and contribute to the mission of Sea Mar Community Health Centers.

Responsibilities:
  • Conduct initial screenings and comprehensive integrated clinical psychosocial assessments to identify client needs and develop care plans.
  • Provide brief treatment and interventions to clients, including counseling and support services.
  • Develop and implement care plans to address client needs and goals.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Provide advocacy and support to clients to overcome barriers and achieve their goals.
  • Maintain accurate and timely documentation of client services and progress.
Requirements:
  • Bachelor's degree from an accredited university and current Certified Counselor through the State of Washington Department of Health or Chemical Dependency Counselor.
  • Experience working with underserved populations, including those with limited English proficiency, homelessness, and migrant and seasonal workers.
  • Knowledge of evidence-based screening tools and ability to use them to inform care decisions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
