Job SummaryAs a Case Manager at Duke University, you will play a vital role in providing professional nursing care to patients in their homes or alternate-site settings. You will work closely with patients, families, and healthcare teams to develop and implement personalized care plans, ensuring seamless transitions and optimal outcomes.
Responsibilities- Assess patient needs and develop comprehensive care plans in collaboration with healthcare teams.
- Provide education and support to patients and families, promoting self-management and empowerment.
- Coordinate care services, including medication management, wound care, and infusion therapy.
- Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
- Document patient progress, outcomes, and care plans in accordance with Duke University's policies and procedures.
- Associate Degree or Diploma in Nursing required; BSN preferred.
- Current North Carolina RN licensure or compact RN licensure in participating state.
- 2+ years of med-surgical, critical care, or home health/hospice/infusion experience.
- Valid driver's license and current automobile insurance coverage.
