Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at Motion Recruitment Partners. As a Case Manager, you will play a critical role in coordinating care, resources, and services for our members to achieve optimal clinical and resource outcomes.

Responsibilities:
  • Collaborate with multidisciplinary teams to develop and implement comprehensive care plans for members.
  • Conduct thorough assessments to identify member needs, health status, and educational status.
  • Develop and implement member-centric plans tailored to individual needs, health status, and level of support required.
  • Utilize community resources and funding sources to support member care plans.
  • Monitor and manage member care, including scheduled follow-up, plan discussion, and referrals as needed.
  • Evaluate and facilitate care provided to members through the continuum of care.
  • Document all aspects of the plan, including initial assessment, development, implementation, monitoring, and evaluating outcomes.
Requirements:
  • RN degree with 5-7 years of experience or RN, RD, or LCSW with 3 years of clinical and/or case management experience.
  • Valid and applicable clinical license (NC or compact multi-state licensure) to perform job duties.
  • Additional specialty certification (i.e. CCM, CDCES) may be required for some roles.
