Job SummaryThe Case Manager will be responsible for assessing the needs of program participants, developing and implementing individualized service plans, and connecting them with community resources. This is a dynamic and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Key Responsibilities- Conduct comprehensive assessments of program participants to identify their strengths, needs, and goals.
- Develop and implement individualized service plans that address the unique needs and goals of each participant.
- Connect participants with community resources, including healthcare services, housing, employment, and education.
- Provide ongoing support and advocacy to participants, including crisis intervention and case management.
- Collaborate with other service providers and community agencies to ensure a comprehensive and coordinated approach to service delivery.
- Bachelor's degree in Human Services or a related field.
- Intermediate computer skills, including proficiency in case management software.
- One year of case management experience preferred.
- Valid driver's license.
This is a full-time position that requires working in a community-based setting. The Case Manager will be required to drive program participants to appointments and may be required to work flexible hours, including evenings and weekends.
