Position Objective:

The Registered Nurse (RN) Navigator-Oncology plays a crucial role in orchestrating patient care for specific tumor-site cancer populations. This position operates independently while collaborating with various healthcare professionals to serve as a patient advocate, educator, and consultant. The RN Navigator-Oncology is dedicated to achieving optimal clinical outcomes by effectively managing patient care throughout the cancer journey and across diverse healthcare environments, utilizing resources in a strategic and efficient manner. The responsibilities are executed in a way that fosters a multidisciplinary team approach. The patient care process—comprising assessment, planning, implementation, and evaluation—is applied to patients across the tumor-site spectrum. The RN Navigator-Oncology is committed to upholding patient safety and implementing relevant safety practices. Patient and family-centered care is prioritized, ensuring age-appropriate services are delivered. This role requires maintaining professional competencies through participation in educational programs and professional organizations.

Essential Responsibilities

The following responsibilities are not exhaustive but highlight the primary tasks required:
  • Exhibit comprehensive understanding of the pathophysiology associated with specific diseases.
  • Grasp oncology treatment modalities and their rationale, including surgery, chemotherapy, biotherapy, radiation therapy, symptom management, survivor care, and follow-up.
  • Employ current clinical guidelines to assist in formulating and executing treatment plans (e.g., NCCN guidelines).
  • Collaborate with healthcare providers and patients to manage physical, mental, psychosocial, and spiritual well-being.
  • Evaluate current health status to address health promotion needs, functional status, development, and lifestyle factors to enhance health outcomes.
  • Contribute to the creation of treatment plans tailored to patient needs and aimed at optimizing disease prognosis/outcomes.
  • Implement a teaching-coaching approach when engaging with patients:
    1. Assess patient comprehension of the care plan;
    2. Assist patients in understanding medical terminology;
    3. Utilize effective communication skills, including active listening.
  • Ensure continuity of care throughout the entire cancer experience, from pre-diagnosis to survivorship, palliative/hospice care, and end-of-life considerations.
  • Develop and recommend educational resources for patients regarding their diagnosis and disease for tumor-site populations.
  • Identify implications and availability of resources at local, regional, and national levels to support a comprehensive and reasonable continuum of care.
  • Assist in coordinating, implementing, evaluating, and modifying the overall care plan through an interdisciplinary and collaborative team process to ensure timely and comprehensive service coordination.
  • Facilitate communication among providers and physician practices to build strong relationships.
  • Engage in Oncology-specific committees.
  • Evaluate patient ability to navigate the healthcare system and identify potential barriers (e.g., continuity, coordination of care, and communication with the healthcare team).
  • Guide patients through the healthcare system, including appropriate referrals to social work, dietitians, and insurance advocates.
  • Manage orders, ensuring patients receive appropriate referrals to surgeons, genetic counselors, radiation oncology, medical oncology, chemotherapy, imaging, labs, etc.
Other Responsibilities
  • Perform all other related duties as assigned in a professional and responsive manner.
  • Demonstrate proficiency in using relevant equipment.
  • Support, understand, and promote Alomere Health's mission, vision, values, policies, and procedures.
  • Exhibit flexibility in meeting work schedule requirements based on departmental needs.

Required Qualifications
  • Minimum of 3 years clinical experience in Oncology.
  • Active and current MN RN Licensure.
  • BLS certification.
  • Experience and skills in patient education.
  • Experience working within multidisciplinary teams.
Preferred Qualifications
  • Bachelor's Degree or higher in Nursing.
  • Experience with Epic systems.
  • Experience in Health Care Home and Care Coordination.
  • Commission on Cancer approved national oncology certification.
