- Monitor and Evaluate Care: Continuously monitor and evaluate patient care to ensure that it is meeting individual needs and optimizing disease outcomes.
- Education: Bachelor's degree in Nursing or higher
- Experience: 3 years minimum clinical experience in Oncology
- Licensure: Active and Current MN RN Licensure
- Certifications: BLS and Commission on Cancer approved national oncology certification
- Skills: Strong verbal and written communication skills, strong oncology knowledge, critical thinking skills, interpersonal skills, strong organizational skills, and ability to prioritize and reprioritize quickly
- Epic Experience: Experience with Epic electronic health record system
- Healthcare Home and Care Coordination: Experience in healthcare home and care coordination
