Key Responsibilities

The Community Support Specialist is tasked with implementing care standards for social work services within the home health and hospice environment.

Core Duties Include:

- Evaluating the psychosocial status of clients in relation to their health conditions and surroundings, and sharing insights with the nursing team.
- Conducting social assessments and devising intervention strategies based on evaluation results.
- Maintaining accurate clinical documentation for all clients referred to social work services.
- Providing information and referral assistance to clients and their families regarding community resources and needs.
- Promoting agency programs and events within the local community.
- Acting as a liaison between clients, families, and community organizations.
- Fostering collaborative relationships with organizational staff to enhance client care.
- Developing and maintaining partnerships with public and private agencies to support client needs.
- Participating in the formulation of comprehensive care plans and case discussions as necessary.
- Assisting the medical team in recognizing significant social and emotional factors affecting health issues.
- Engaging in discharge planning processes.
- Coordinating volunteer training, ongoing education, and annual assessments.
- Assigning volunteers to hospice families and managing visit schedules.
- Overseeing the bereavement support program.
- Collaborating with the agency director as the primary contact for the foundation department at the local hospital.
- Managing event planning and execution.
- Ensuring proper documentation of social services in compliance with relevant standards.
- Performing additional duties as assigned by the Clinical Director/Supervisor.

Qualifications

- Bachelor's degree in Social Work
- Licensed Social Worker in the relevant states
- Minimum of one year of experience in a healthcare setting
- Strong verbal and written communication skills, along with excellent organizational abilities
- Basic proficiency in computer applications

Preferred Qualifications:
- Master's degree in Social Work (salary commensurate with experience)
- Experience in Home Health and Hospice settings
