Patient Intake Coordinator

The Patient Intake Coordinator is responsible for placing new and existing patients with Fresenius Kidney Care (FKC). This role requires excellent sales and customer service skills, as well as the ability to work in a fast-paced environment managing an average case load of 75 – 100 patients.
Key Responsibilities:
• Coordinating with referral sources, including hospital discharge planners, FKC clinics, and non-Fresenius clinics, to secure schedules and ensure patients receive financial and medical clearances for lifesaving dialysis from FKC.
• Managing cases in Salesforce CRM while receiving and making multiple phone calls to place patients.
• Serving as the primary liaison with referral sources, clinics, and discharge planners to obtain chair times, financial clearance, and medical clearance in a timely manner to expedite admissions.
• Collaborating with facility staff and management to effectively resolve issues impacting patient admissions.
• Communicating with individual clinics as needed to provide prompt scheduling of patients and forwarding necessary documents and records to receiving facilities in a timely manner.
Requirements:
• 2 – 4 years' related experience.
• Excellent Sales & Customer Service, Communication, Organizational, and Interpersonal skills required.
• Detail-oriented with the ability to enter information accurately into a database system.
• Working knowledge of standard office applications – email, word processing, spreadsheets, and presentations.
• Completion of and compliance with the Marketing and Business Development Functional Compliance Program.
• Ability to adapt to change and support new policies and procedures in a positive manner.
