Hillcrest Health Services is seeking a full-time LPN to become an integral part of our Nebraska Hospice team. Our dedicated staff at Hillcrest Hospice Care is passionate about providing holistic, compassionate, and skilled end-of-life care to those in need.

Role Overview:
The Hospice LPN is responsible for delivering direct patient care services, utilizing specialized knowledge and experience in bedside care, symptom management, crisis intervention, and family support.

Daily Responsibilities:

  • Conduct thorough assessments of the physical, functional, psychosocial, and nutritional status of assigned patients in a systematic and ongoing manner to ensure the delivery of necessary patient services.
  • Collaborate professionally with physicians, team members, patients, families, and other caregivers to develop a comprehensive plan of care and coordinate service delivery.
  • Administer nursing services in alignment with the established plan of care, employing nursing processes effectively.
  • Prepare clinical documentation for bi-weekly interdisciplinary meetings to accurately reflect the patient's current condition and communicate any changes to the physician and other team members.
  • Engage in service line meetings and team conferences to ensure that all patient and family needs are prioritized and met appropriately.

Qualifications:

  • Previous experience in hospice or home health care is preferred.
  • Must hold a valid LPN license in the state of Nebraska or a compact state.
  • A minimum of three years of nursing experience is required.
  • Valid state driver's license, current automobile insurance, and reliable transportation are necessary.
  • Proficient computer skills are essential.
  • Must demonstrate compassion and the ability to work independently.
