Hillcrest Health Services is seeking an experienced and organized Registered Nurse (RN) to join our On Call Hospice team.
Job SummaryThis is a full-time position that involves being on call 7 days, followed by 7 days off. The call period is from 5pm to 8am, Monday through Monday. You will work alongside a second RN who will be your teammate, with additional support from the Night Team, Medical Director, Social Services, and the Administrator On-Call.
Responsibilities- Answer calls in a timely and professional manner, with accurate triaging and communication with patients, families, and facilities.
- Assess the physical, functional, psychosocial, and nutritional status of assigned patients in a systematic and continuous manner.
- Make initial visits to patients and families to explain hospice services, evaluate patient appropriateness, admit patients, and initiate a plan of care with authorization from the attending physician and medical director.
- Participate in service line meetings as required.
- Graduate from an accredited nursing program.
- At least three years of experience as a nurse.
- At least two years of experience in hospice or home care setting preferred.
- Licensed in the state of Nebraska or compact state as a Registered Nurse.
- Valid driver's license.
- Reliable means of transportation and current automobile insurance.
- BLS certification by the AHA.
- Health, vision, and dental insurance.
