Patient Care Coordinator

Nova Southeastern University is seeking a highly skilled and detail-oriented Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support services.
Key Responsibilities- Work closely with faculty and healthcare providers to manage patient oral health issues.
- Collect and input data related to patient satisfaction surveys.
- Conduct data entry and generate reports from multiple clinic sites.
- Consult with patients' caregivers, healthcare providers, and social services providers to facilitate access and utilization of services.
- Schedule patient appointments, facilitate transportation, insurance information, and medical record needs.
- Respond appropriately to patient concerns, outside agencies, and other third-party personnel.
- Distribute and retrieve diagnostic data (prescriptions, lab tests, and blood test results).
- Remain apprised of activities, current and planned, with the programs.
- Greet patients and visitors in a courteous and professional manner.
- Check for completeness of records and input all information into computer systems.
- Answer phone calls in an accurate and timely manner, while being professional and courteous.
- Maintain message logs and conduct follow-up phone calls for cancelled and missed appointments.
- Recruit patients for grant-approved programs.
- Assist with residents to supply data for needed projects related to the grant.
- Contact programs' directors of any changes to the residents' schedules at grant clinics.
- Monitor doctor's treatment plans to schedule follow-up appointments.
- Perform necessary office and clerical functions.
- Complete other projects as required.
- High School Diploma or Equivalent.
- Four (4) years' experience in a directly related field, or an Associate's Degree or 60 college credits and two (2) years' experience in a directly related field, or a Bachelor's degree and one (1) year experience in a directly related field.
- Experience in Electronic Health Records (EHR) systems, preferably with AxiUm software.
- Bilingual Spanish - English.
- Strong organizational skills and meticulous record keeping.
- Exceptional people skills, presentation skills, and phone etiquette skills.
- This is a non-safety sensitive position.
- Background screening is required.
