About the Opportunity

Optum is seeking a skilled Nurse Practitioner or Physician Assistant to join its HouseCalls team in New York. This unique role offers an opportunity to provide exceptional patient care in the comfort of their homes.

Role Overview

The ideal candidate will conduct comprehensive in-home assessments, including evaluations of past medical history, reviews of systems, medication reconciliation, vital signs, and physical exams. They will also perform evidence-based practice screenings and communicate findings with members' Primary Care Physicians to inform potential gaps in care.

Responsibilities

  • Conduct home visits to assess patients' health and well-being.
  • Evaluate medical history, review systems, reconcile medications, take vital signs, and perform physical exams.
  • Perform point-of-care testing as needed.
  • Identify diagnoses for care management and active medical management.
  • Communicate assessment findings to PCPs to inform potential gaps in care.
  • Educate patients on disease processes, medication adherence, and compliance.

Requirements

  • Active NP license or ability to obtain by start date.
  • NPs practicing autonomously must have obtained approval from their licensing board.
  • Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain national certification.
  • Ability to complete physical requirements of the job, including lifting 30-pound bags and navigating stairs.
  • Reliable transportation for home visit assessments.

Preferred Qualifications

  • Additional licensure in other geographic areas may be requested.
  • 1+ years of clinical experience in the highest level of education, clinical setting.
  • Experience in gerontology, cardiology, internal medicine, or endocrinology.
  • Home Health care or home visit experience.
  • Ability to travel throughout the state for occasional visits.
  • Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.

Compensation

The estimated annual salary range for this role is $88,000 to $173,200 based on local labor markets, education, work experience, certifications, and performance. Opportunities for career growth and professional development are available within Optum's dynamic environment.
