Physician Assistant Home Risk Assessor

We are seeking a skilled Physician Assistant to join our team as a Home Risk Assessor. As a Home Risk Assessor, you will be responsible for conducting comprehensive health assessments in patients' homes, identifying potential health risks, and providing personalized care plans to improve their overall health.
Key Responsibilities:
- Conduct thorough medical history reviews and physical examinations to identify potential health risks
- Develop and implement personalized care plans to address identified health risks
- Collaborate with healthcare providers and other stakeholders to ensure seamless patient care
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document all interactions and assessments in accordance with company guidelines
Requirements:
- Master's degree in Physician Assistant Studies or equivalent experience
- Current RN and PA licensure in the state of practice
- Board certification by a recognized professional organization (e.g., NCCPA, AANP, ANCC)
- 1+ year of experience as a Physician Assistant in a clinical setting
- Excellent communication and interpersonal skills
- Ability to travel to patient locations and work in a fast-paced environment
