Patient Care Coordinator

We are seeking a skilled and compassionate Home Health Aide to join our team at Frontier Home Health and Hospice. As a Home Health Aide, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
Responsibilities- Carry out assigned tasks on clients and follow Home Health Aide Care Plans, performing delegated tasks after instruction by a Registered Nurse (RN) or Physical Therapist (PT).
- Bathe clients, assist with showers and partial baths as appropriate, promoting personal hygiene and dignity.
- Assist clients with proper exercises and collaborate with PT in rehabilitation efforts when applicable, enhancing mobility and independence.
- Help clients maintain personal hygiene and assist with all aspects of activities of daily living, including grooming, dressing, and feeding.
- Provide education and support to clients and their families on self-care techniques, empowering them to take an active role in their care.
- Assist clients in preparing for doctor appointments, ensuring timely and efficient care.
- Lift, turn, and weigh clients as necessary, exercising caution and care.
- Take and record vital signs, including blood pressure, temperature, and respiration, as assigned.
- Certified Nursing Assistant (CNA) certification is strongly preferred.
- Registration and listing in good standing on the Nurse Aide Registry in the state of employment.
- Minimum of 1 year of experience in a home health or hospice setting.
- Minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility).
- Ability to recognize the needs of people, maintain good relationships, and communicate effectively with supervisors.
