Care Coordinator

We are seeking a skilled Care Coordinator to join our team at WMC Health. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of efficient and cost-effective healthcare services.
Key Responsibilities- Conduct ongoing quality monitoring, identifying quality, risk management, and infection control issues, and report them to the Director of the appropriate department.
- Assess patients within the caseload to identify needs, issues, resources, care goals, discharge needs, and beyond if necessary.
- Plan the patient's care collaboration with the healthcare team, patient/family, for the hospital stay, and communicate continually with physicians.
- Coordinate the care of patients whose care is at risk from either a clinical or cost perspective.
- Initiate the discharge planning assessment on admission and coordinate discharge/post-discharge activities and plan.
- RN, Graduate of an accredited school of Nursing or BSW/CSW (BSW minimum) or Masters in another health-related field.
- 2 years hospital-based experience, with knowledge of medical/surgical cases, and one (1) year QA/UR or Case Management preferred.
- PRI certification preferred.
