Job Title: Care Manager

We are seeking a skilled Care Manager to join our team at Quality Private Duty Care. As a Care Manager, you will play a vital role in ensuring the delivery of high-quality patient care and services.

Key Responsibilities:
  • Assist with calls or inquiries from referral sources and provide accurate information regarding home health care requirements.
  • Obtain patient insurance information and conduct initial admission visits to ensure timely service initiation.
  • Complete supporting documentation, including referral forms, consent/service agreements, and OASIS assessments.
  • Establish and maintain effective primary care teams and schedule in-home staff according to the plan of care.
  • Coordinate referrals for MSS and home health care services as needed.
  • Supervise and evaluate services provided by RNs, LPNs, CNAs, and HMAs in accordance with the plan of care and organizational policies.
  • Develop and revise personal care plans as needed.
  • Perform case management visits as ordered by the MD to improve patient care quality.
  • Provide frequent communication to physicians, in-home staff, and others to coordinate changes in patient care and ensure proper documentation.
  • Coordinate requests for oxygen, medical equipment, pharmacy needs, and community services according to patient need and physician order.
Requirements:
  • Registered Professional Nurse (RPN) license in the state of Tennessee.
  • Minimum two years of home health experience.
  • Working knowledge of the home health care program and previous management/supervisory experience preferred.
  • Previous experience scheduling home health visits preferred.
  • Ability to provide direct patient care and take on-call assignments.
  • Ability to travel, with trips varying in length and potentially requiring overnight stays.
  • Computer data entry and word processing skills desirable.
