About the Role

We are seeking a highly skilled Care Services Coordinator to join our team at Arkos Health. As a Care Services Coordinator, you will play a critical role in facilitating care for patients by coordinating and scheduling with our field providers.

Key Responsibilities
  • Provide phone support to transitional/post-acute teams through inbound/outbound call management of patient needs
  • Explain services and obtain verbal consent from patients for transitional/post-acute team and clinical services
  • Coordinate and manage schedules for transitional/post-acute and clinical services teams in accordance with contracted agreements
  • Initiate timely patient contact, obtain required documentation, and scheduling for the department
  • Act as a liaison with referral sources, transitional/post-acute team, clinical services, and health plans for coordination of orders, supplies, appointments, and medical records
  • Upload orders, field documents, and requested records to patient charts for clinical services team
Requirements
  • High school diploma or GED required
  • Some college or medical certification preferred
  • Previous customer service knowledge preferred
  • MA/CNA preferred but not required
  • Healthcare experience preferred
  • Knowledge of medications, medical terminology, and patient care preferred
  • Bilingual is a huge plus
